Return of Income Statement Cards

Statement of Income cards are returning for all our ongoing Ontario Works (OW) clients.

ALL OW clients are required to visit the band office to sign their return cards from August 17^th to 25^th, 2023. Any returns received after August 25^th will be considered late, so please return on time to ensure you receive your social assistance on welfare day.

Statement of Income return cards are available at the main reception area. A drop-off box is also located at main reception.

Income stubs must be reported and included with your return card. If you receive stubs by email from your employer, you can email them to your caseworker. Please print your name on the return card, and sign and date it on the day you return your card.

Also ensure you drop off your hydro bills when you receive them in the mail each month and have them photocopied for your caseworker each month so we can make a payment on your hydro account.

The blue sheets are for Leona Chilton and green sheets are for Edna Sutherland. Make sure you know who your caseworker is.
